Secure Client Portal Terms and Policies
REGISTERING WITH THE SECURE CLIENT PORTAL

All clients must register with the Open Door Therapy Secure Client Portal before receiving services.

Registering requires choosing a username and password as well as completing a general information form and electronically signing it.

CONSENT TO TREATMENT

All clients must sign a form consenting to counseling services with Open Door Therapy,LLC. The form is located in the Secure Client Area and is available during the registration process.


PAYMENT POLICY: FEE-FOR-SERVICE METHOD

Clients pay for counseling sessions at the time that services are delivered.


TREATMENT EXPECTATIONS & LIMITATIONS

The expectation is that clients partake in treatment until resolution to the problem or until the client feels equipped enough to remedy issue on his or her own.

ELIGIBILITY

Clients must be at least 18 years-old to register.



Nature of Counseling:

There may be both benefits and risks while participating in counseling, distance or otherwise. Counseling may improve your ability to relate with others, provide a clearer understanding of yourself, your values, and your goals. Since counseling may also involve discussing unpleasant aspects of your life, you may also experience uncomfortable feelings. Counseling often leads to better relationships, solutions to specific problems, and significant improvement in feelings of distress.


However, please understand there are no guarantees of what you will experience, and that you enter this Agreement and use Terri DiMatteo, LPC’s services at your own risk. You agree that you understand the possible advantages and disadvantages of online therapy and shall not hold accountable Terri DiMatteo, LPC for any information or insight distributed here.


Agreement
This Agreement shall be interpreted only in accordance with the laws of the State of New Jersey and any legal proceeding associated with this Agreement will occur exclusively in the courts located in New Jersey.


Limits of Confidentiality:
There are some situations in which practitioners are legally and ethically obligated to take actions they believe may be necessary to protect a client or others from harm. If such a situation arises, Terri DiMatteo, LPC will make every effort to fully discuss the issue with the client before taking any action and will limit disclosure to only what is necessary.


If a practitioner has reason to believe that a child or vulnerable adult is being neglected or abused, the law requires that the situation be reported to the appropriate state agency.


If a client presents a clear and substantial danger of harm to himself/herself or others, Terri DiMatteo, LPC is ethically obligated to take protective actions.


These actions may include contacting family members, assisting with hospitalization, notifying any potential victim(s), and notifying the police.


If you are considering suicide, or believe you yourself to be a potential safety threat to others, you must immediately call 911, (800) LIFE-NET and/or notify the police and/or seek emergency care at your local hospital.


Additionally, Terri DiMatteo, LPC will file a formal report if a potential online clients states any desire to do harm to him/herself.


The policies govern -- among other things -- the manner in which Open Door Therapy collects, uses, maintains and discloses information collected from users (each, a "User") of the www.opendoortherapy-telehealth.net website ("Site"). This privacy policy applies to the Site and all products and services offered by Open Door Therapy.

Personal identification information

I may collect personal identification information from Users in a variety of ways, including, but not limited to, when Users visit our site, place an order, respond to a survey, fill out a form, and in connection with other activities, services, features or resources we make available on our Site. Users may be asked for, as appropriate, name, email address, mailing address, phone number. Users may, however, visit our Site anonymously. We will collect personal identification information from Users only if they voluntarily submit such information to us. Users can always refuse to supply personally identification information, except that it may prevent them from engaging in certain Site related activities.

Non-personal identification information

I may collect non-personal identification information about Users whenever they interact with our Site. Non-personal identification information may include the browser name, the type of computer and technical information about Users means of connection to our Site, such as the operating system and the Internet service providers utilized and other similar information.

Web browser cookies

Our Site may use "cookies" to enhance User experience. User's web browser places cookies on their hard drive for record-keeping purposes and sometimes to track information about them. How we use collected information
Open Door Therapy may collect and use Users personal information for the following purposes:

- To personalize user experience: I may use information in the aggregate to understand how our Users as a group use the services and resources provided on our Site.

- To send periodic emails: I may use the email address to send User information and updates pertaining to their order. It may also be used to respond to their inquiries, questions, and/or other requests. If User decides to opt-in to our mailing list, they will receive emails that may include company news, updates, related product or service information, etc.


How Your Information is Protected

Iadopt appropriate data collection, storage and processing practices and security measures to protect against unauthorized access, alteration, disclosure or destruction of your personal information, username, password, transaction information and data stored on our Site.

Sensitive and private data exchange between the Site and its Users happens over a SSL secured communication channel and is encrypted and protected with digital signatures.

Sharing your personal information

I do not sell, trade, or rent Users personal identification information to others. We may share generic aggregated demographic information not linked to any personal identification information regarding visitors and users with our business partners, trusted affiliates and advertisers for the purposes outlined above.

Users may find advertising or other content on our Site that link to the sites and services of our partners, suppliers, advertisers, sponsors, licensors and other third parties. We do not control the content or links that appear on these sites and are not responsible for the practices employed by websites linked to or from our Site. Browsing and interaction on any other website, including websites which have a link to our Site, is subject to that website's own terms and policies.

Compliance with children's online privacy protection act

Protecting the privacy of the very young is especially important. For that reason, we never collect or maintain information at our Site from those we actually know are under 13, and no part of our website is structured to attract anyone under 13.

Open Door Therapy has the discretion to update these policies at any time. When we do, we will post a notification on the main page of our Site, revise the updated date at the bottom of this page and send you an email. We encourage Users to frequently check this page for any changes to stay informed about how we are helping to protect the personal information we collect. You acknowledge and agree that it is your responsibility to review this privacy policy periodically and become aware of modifications.
